Sepsis Prevention
Sepsis is a severe reaction to infection that can quickly become life threatening. Sepsis can increase the length of hospital or facility stays, and can have a devastating effect on the patient, including physical disability, organ damage or cognitive impairment.
Springhill Home Health & Hospice is vigilant when it comes to sepsis prevention, implementing our “best practice” program to educate and train all staff in sepsis prevention, recognition, intervention and treatment.
With these prevention measures in place, Springhill Home Health & Hospice has been able to effectively decrease the number of sepsis occurrences and sepsis-related hospital readmissions for our patients. Our program also includes specialized care for those who are admitted to home health following hospitalization for sepsis, helping them to attain the optimum level of health and functioning possible.
Heart Health
Congestive heart failure is a chronic condition in which the heart does not pump efficiently. Patients with CHF are at risk for an increase in cardiac symptoms and subsequent hospitalizations. Our special cardiac team uses a multifaceted approach to stabilize the patient’s condition, reduce re-hospitalizations, increase independence and improve overall quality of life.
COPD Care
Springhill’s Pulmonary Care program centers on patients with lung disease, such as chronic obstructive pulmonary disease, or COPD. COPD is a debilitating lung disease that affects more than 11 million people in the United States alone. COPD makes breathing difficult and can cause wheezing, fatigue, respiratory infections, pain and discomfort. Since COPD is a chronic condition, it requires long-term treatment.
Springhill’s COPD health care team works with each patient to develop a step-by-step symptom management plan. In addition to delivering prescribed medical treatments, we educate patients about self-care, medication, nutrition, breathing techniques, oxygen safety and more. Services allow patients to stay at home, avoiding frequent re-hospitalizations and optimizing comfort and independence.
Orthopedics
Springhill Home Health is proud of our team of orthopedic specialists. They combine years of experience and clinical expertise with the most recent technology to create an advanced orthopedic program. The program addresses recovery from surgery, pain control and comprehensive rehabilitation. Our team evaluates all aspects of a patient’s daily life to create an effective, personalized care plan to help patients achieve the highest level of functioning and independence possible.
Joint Replacement and Amputee Care
Our orthopedic program includes specific program tracks that offer advanced expert care to patients with certain conditions such as those recovering from joint replacement surgery or those who have had a limb amputated. Springhill Home Health takes a team approach to orthopedic care that includes a variety of professionals, each bringing a unique perspective and together providing holistic care.
